School and nursery health check

school health check · school medical form · nursery medical certificate · kindergarten health check

A pediatrician and other doctors examine your child and fill in a medical record for school or nursery. Vaccines and general health are checked.

Before starting nursery or school for the first time, or when moving to another one.

⏱ Duration: usually a few days or weeks to see all doctors and do tests; the pediatrician completes the form at the last visit

Be sure to tell your doctor

How to prepare

  • Start early. Begin 1 to 2 months ahead, so you can see all the doctors without rush.
  • Checklist. Ask the clinic for the list of doctors and tests. It depends on age.
  • Your child. Tell your child the doctors will look and listen, without scary words.

What to bring

  • Your child's documents and yours
  • The vaccination record
  • Your child's medical record
  • The form from the school or nursery, if given

How it goes

  1. The pediatrician gives you a list of doctors and tests
  2. Your child has the tests
  3. You see the specialists: usually an eye doctor, ENT, neurologist, surgeon, and dentist
  4. The pediatrician checks vaccines
  5. The pediatrician collects the reports and completes the medical record

What you'll feel

  • The exams are painless
  • Only the blood test is unpleasant

Afterwards

  • Give the medical record to the school or nursery nurse
  • If vaccines are due, they are given on schedule
  • If a doctor found something, you will be told what to do next

Common questions

Will my child be accepted without vaccines?

Rules depend on the country and the school. Check with your clinic and the nursery.

Can it all be done in one day?

Sometimes, yes. Some clinics have such programs. Ask in advance.
